What Is a Home Sleep Apnoea Test?

A home sleep apnoea test (HST) is a portable sleep study performed in your home. It records breathing patterns, oxygen levels, snoring and respiratory effort.

Compared with in-lab polysomnography (PSG), a home test is more straightforward, convenient and suitable for patients with a high likelihood of OSA. However, PSG may still be required for complex cases or when other sleep disorders are suspected.

Types of HST devices:
  • Type II: Comprehensive Testing – Records multiple signals, including brain waves, eye movements, muscle tone, heart rate, airflow and oxygen levels, similar to an in-lab study.
  • Type III: Portable Devices – Focuses on key respiratory measures such as airflow, snoring, breathing effort, oxygen saturation and heart rate. Easier to use and more common for home testing.

Local Guidelines & Eligibility in Singapore

According to ACE-HTA and MOH guidance, a home sleep test is recommended for adults (≥18 years) who:

  • Have high pre-test probability of moderate-to-severe OSA, with symptoms such as:

    • Excessive daytime sleepiness, plus
    • At least two of: loud snoring, witnessed apnoeas/gasping/choking, or hypertension.

  • Do not have complicating conditions such as significant cardiopulmonary disease, neuromuscular disorders, or severe insomnia.

Important: A credentialled sleep medicine specialist must interpret all HST results to ensure accuracy and quality of care.

  • Older age: The risk of OSA increases with age.
  • Male sex: Men are more prone to OSA, though women are also affected, especially after menopause.
  • Anatomical airway issues: Narrow airways, enlarged tonsils, or structural differences can contribute to sleep apnoea.

When HST Is Not Suitable

While convenient, home sleep tests may not be the right choice for everyone. You may require an in-lab study if you have:

  • Serious comorbidities: Conditions like severe heart or lung disease may complicate results.
  • Suspected central sleep apnoea: HST is less accurate for detecting this condition.
  • Neuromuscular disease: Disorders affecting muscle control can interfere with breathing during sleep.
  • Other sleep disorders such as parasomnias: Conditions like sleepwalking or night terrors require more detailed monitoring.

Despite the popular use of Body Mass Index (BMI) in clinical practice, it is a poor tracking tool for weight change—there’s no way to find out if changes in your weight are in fat or muscle by BMI alone.

On the other hand, a body composition assessment can tell you how much of your overall weight loss is due to either water, fat, or muscle loss.

Whatever your goals may be, an important thing to remember during your health journey is that there’s more to being healthy than looking thin.

How the Home Sleep Test Works (Process)

  1. Consultation & Prescription: Your doctor will review your sleep-related symptoms, medical history and risk factors to confirm your suitability for a home test.
  2. Device Pickup or Delivery: A portable monitoring device will be provided. You can collect it at the clinic or have it delivered to your home for convenience.
  3. Setup Instructions & Training: Our staff will guide you through the setup in person or via a step-by-step video call, so you feel confident using the device.
  4. Overnight Monitoring: Wear the device while you sleep in your own bed. It records key data without disrupting your rest.
  5. Device Return / Data Upload: Return the device to the clinic or arrange for a courier to collect it. The recorded data will be securely uploaded for review.
  6. Analysis & Report: A sleep specialist will interpret your results, focusing on breathing patterns, oxygen levels and other signals that may indicate sleep apnoea.
  7. Follow-Up: You will discuss your results and possible treatment options, including a CPAP trial, lifestyle changes, or further testing.

Possibility of Incomplete or Invalid Data

Since the device is self-administered, sensors may become dislodged during the night, which can affect the accuracy of the results.

Not Suitable for Patients with Serious Comorbidities

Individuals with conditions such as severe heart disease, advanced lung disorders, or neuromuscular disease often need closer monitoring in a sleep laboratory.

If you are diagnosed with moderate to severe sleep apnoea, your ability to drive safely may be affected, especially if left untreated. Always follow your doctor’s recommendations before resuming driving.

Sometimes results may be unclear due to sensor issues or borderline findings. In such cases, your doctor may recommend repeating the HST or proceeding with an in-lab polysomnography for a more detailed study.
